AV/TECH Request Form

This form is used to request use of AV/TECH equipment or assistance with tech for CHURCH-RELATED ACTIVITIES ONLY (classes, meetings, events, worship, etc). Please be sure to check the church calendar for date, time, and meeting location conflicts before making your request. For any event that is not church-related, such as a wedding or memorial service, please do not use this form. We encourage you to contact our Congregational Administrator at administrator@firstuunash.org or 615-383-5760 for non-church-related events. Thank you.

Please note that any event requiring the use of an audio mixer, speaker, and/or microphone will require assistance from a trained FUUN staff or volunteer member. Please be sure that you complete this request at least two weeks before your event. If the event is less than two weeks away, it is not guaranteed that you will be able to receive assistance.

AV/TECH Needs

Note: We need at least one week to fulfill requests. Preferably, it's best to let us know needs as early as possible for scheduling purposes. Access to worship equipment CANNOT BE GUARANTEED for any given event, but we will do our best to be accommodating and we will confirm details prior to the event.

Please check which Audio/Visual option you are seeking: Tier 1: You want to be able to connect a computer to a tv screen to display it for a small group in person only event and/or meeting. Equipment required: TV , hdmi cable, USB hub adapter (computer not provided) Tier 2: You are hosting a hybrid or multi-platform event and/or meeting with no more than ten people attending in person. Equipment required: TV , hdmi cable, OWL Camera, USB hub adapter (computer not provided) You are hosting a hybrid or multi-platform event and/or meeting in the Social Hall of a moderate size. Equipment required: TV, hdmi cable, OWL Camera, USB hub adapter, audio mixer, speaker, microphone Tier 4: You are hosting a multiplatform or hybrid event and/or meeting in the Sanctuary with full audio and video capabilities. Equipment required: Mixing Board, PTZ camera, camera controller, video switcher Option A: You are hosting an in person only event in the Social Hall where you would like a Microphone to be able to speak to a bigger audience or group.
