AV/TECH Request Form

This form is used to request use of AV/TECH equipment or assistance with tech for CHURCH-RELATED ACTIVITIES ONLY (classes, meetings, events, worship, etc). Please be sure to check the church calendar for date, time, and meeting location conflicts before making your request. For any event that is not church-related, such as a wedding or memorial service, please do not use this form. We encourage you to contact our Congregational Administrator at administrator@firstuunash.org or 615-383-5760 for non-church-related events. Thank you.

Please note that any event requiring the use of an audio mixer, speaker, and/or microphone will require assistance from a trained FUUN staff or volunteer member. Please be sure that you complete this request at least two weeks before your event. If the event is less than two weeks away, it is not guaranteed that you will be able to receive assistance.

Access to worship equipment CANNOT BE GUARANTEED for any given event, but we will do our best to be accommodating and we will confirm details prior to the event.
