Calendar & Campus Use Request Form

This form is used to request space and add events to the calendar for CHURCH-RELATED ACTIVITIES ONLY (classes, meetings, events, worship, a table in the social hall, lawn use for events/activities, etc). This form should be completed even if your event is on Zoom, held off campus, or is closed to additional participants. Please be sure to check the church calendar for date, time, and meeting location conflicts before making your request. For any event that is not church-related, such as a wedding or memorial service, please do not use this form. We encourage you to contact our Congregational Administrator at administrator@firstuunash.org or 615-383-5760 for non-church-related events. Thank you.
