Calendar & Campus Use Request Form

This form is used to request space and add events to the calendar for CHURCH-RELATED ACTIVITIES ONLY (classes, meetings, events, worship, a table in the social hall, lawn use for events/activities, etc). This form should be completed even if your event is on Zoom, held off campus, or is closed to additional participants. Please be sure to check the church calendar for date, time, and meeting location conflicts before making your request. For any event that is not church-related, such as a wedding or memorial service, please do not use this form. We encourage you to contact our Congregational Administrator at administrator@firstuunash.org or 615-383-5760 for non-church-related events. Thank you.

****Please note that this form no longer contains options to request AV/TECH equipment assistance or childcare. If you need to request use of church AV/TECH equipment, request AV/TECH assistance, or request childcare for your event, please complete the AV/TECH Request Form or the Childcare Request form. In addition, both forms are available under the Members tab on the website.****

If you would like this event/meeting/class advertised on the website, social media, Order of Service, Weekly E-blast, etc., be sure to complete the Get the Word Out Form.

This questions asks about table needs, food and drink, as well as libations. Please note that if you plan to serve alcohol, you must abide by the church's alcohol policy linked for you review.
