Calendar & Campus Use Request Form

This form is used to request space and add events to the calendar for CHURCH-RELATED ACTIVITIES ONLY (classes, meetings, events, worship, a table in the social hall, lawn use for events/activities, etc). This form should be completed even if your event is on Zoom, held off campus, or is closed to additional participants. Please be sure to check the church calendar for date, time, and meeting location conflicts before making your request. For any event that is not church-related, such as a wedding or memorial service, please do not use this form. Please provide a description of your event that will be SHOWN ON THE CHURCH CALENDAR. This should say what you would like people to know. If you would like this event/meeting/class advertised on the website, social media, Order of Service, Weekly E-blast, etc., be sure to complete the Get the Word Out Form.

Will alcohol be present? Having alcohol present MUST comply with the church alcohol policy. A liquor license must also be requested through the church office at least 6 weeks prior to the event being held. Yes No

Do you need Audio/Visual equipment? Note: We need at least 48 hours to fulfill requests for AV equipment prior to the event and access to worship equipment CANNOT BE GUARANTEED for any given event, but we'll do our best to be accommodating and we'll confirm details prior to the event.
